Chronic Disease Management: Hypertension, diabetes (prediabetes counseling, glucose monitoring), hyperlipidemia (high cholesterol), thyroid disorders, obesity & weight counseling, osteoporosis prevention & screening, arthritis & joint pain.
Acute & Sick Visits: Cold, flu & COVID-19 evaluations; sinus & ear infections; upper respiratory infections/bronchitis; urinary tract & bladder infections; rashes, skin infections, abscess; minor eye infections; care for sudden illnesses and minor injuries.
Diagnostic Testing & Monitoring: Remote blood pressure monitoring, diabetes screening (blood sugar, A1C), urinalysis (UTI, protein, sugar, pregnancy tests), cholesterol & lipid panels.
Mental & Behavioral Health: Depression & anxiety screening and management, ADHD/ADD evaluations, medication management, smoking cessation, alcohol & substance abuse counseling, stress & sleep disorder evaluations.
Women’s Health & Family Planning: Contraception counseling, hormone & menopause management, STD testing & treatment, urinary incontinence & pelvic health, breast cancer risk assessment, prenatal counseling (preconception visits).
Men’s Health: Testosterone replacement counseling, erectile dysfunction evaluation & treatment, prostate health screening.
Preventive Screenings & Counseling: Cancer screenings, heart disease risk assessment, metabolic syndrome & obesity screening, sleep apnea risk assessment, dietary & nutritional counseling, lifestyle & preventive coaching, fall risk assessment.
Lab Services: A1C, CMP, CBC, lipid panel, TSH, ACR, liver enzymes, urinalysis, STI testing.
Imaging Services: X-rays, ultrasounds, bone scans, mammograms.
Non-Member Services
Acute & Sick Visits: Care for sudden illnesses and minor injuries, cold/flu/COVID-19 evaluations, sinus infections & allergies, ear infections, upper respiratory infections/bronchitis, urinary tract & bladder infections, rashes/skin infections/abscess, minor eye infections (pink eye, styes).
Medical Consultation: Second opinions, lab reviews, prescription refills (limit 2).
